NHIS CAPTURING IN JAJA CLINIC

This is to inform staff members of the University of Ibadan who booked for NHIS Registration (in response to the Bulletin 4399 Special Release 22 June, 2021) that there will be data capturing as follows:



Date: Tue 07-Thursday 09 September 2021

Venue: NHIS Bldg, Jaja Clinic

Time: 9am-2pm daily

Requirements:

- Staff ID card;

- Photocopy of appointment letter;

- Photocopy of 3 (three) months recent payslip;

- 1 passport picture each for principal & dependant(s).



NHIS regrets the inconvenience, an urgent issue came up for Monday.
